Skilled Nursing
Our registered nurses visit our clients’ homes to perform various health observations and assessments to identify the progression of condition prior to administering care. Aside from providing skilled care, they also educate and ensure our patients and primary caregivers understand the ailments, safety conditions, and other necessary preventive measures needed in the home. They conduct follow-up visits after creating an initial care plan to ensure proper management and effectiveness of care.
Private Duty Nursing
We offer private duty nursing services to ensure patients recovering from or dealing with medical conditions receive the best care possible. With the help of our private duty nurses, adults or children patients who are technologically assisted not only excel in their physical recovery but emotionally and mentally as well. Our nurses also provide patients and their families the necessary support needed to ensure a peaceful atmosphere in the comfort of their own homes.
Therapy Services
Our team also offers rehabilitative services that include physical, occupational, and speech therapies. We have affiliate therapists who are well qualified and trained in helping patients restore physical and emotional strength when they are initially discharged from hospitalization. They use the latest physical, occupational, and speech therapeutic techniques to bring sound health and emotional stability to all of our patients.
Consumer Directed Facilitation Services
We offer primary caregivers the opportunity to provide care to their loved ones without the assistance of a home care agency by providing them with extensive in-house training and facilitation to ensure they are able to provide care themselves under the direction and supervision of our nurses. Consumer Directed Facilitation Services also aim to ensure that families and primary caregivers can handle the complexities of caring for their loved ones. Our registered nurses educate patients and their families on the level and progress of their conditions and how to better manage their current state. They also present many ways our patients can prevent certain diseases and follow up on subsequent visits to evaluate care plan, medications, and effectiveness of care.
Medical Social Work
Our team works with affiliates and partners who provide psychological evaluations of our patients and connect them to counseling resources within the community.
